Questions About Services

​

What conditions qualify for a home visit:  If you have difficulty getting to a doctor's office, then you may qualify for a Provider home visit.  If you are home bound, use assistive devices such as a wheelchair or walker, have physical or cognitive problems when leaving home, or need another person to assist you when leaving home, you may qualify for a Provider home visit.  Contact your health plan to confirm your eligibility.  

 

What if I do not have insurance:  If you do not have insurance, you can pay us directly.​

 

Do I need a referral for a home visit:  You may speak with your health plan to see if a home visit is covered or if a referral is needed to cover cost.

Healthcare clinicians may call and schedule home visits for their patients such as Hospitals, Assisted Living Facilities, Home Healthcare Facilities, etc.​

 

Do you accept Private Pay:  Yes, we do accept Private Pay. We accept credit cards, debit cards and is payable at time of service. 

 

Do you provide Emergency Services:  No, we do not provide Emergency Services. If you are having a medical emergency, call 911 or visit your local hospital.​

 

How do I pay for services:  We accept Medicare, Medicaid, and Insurance.  We accept all major credit cards and debit cards. You will make payment before the provider travels to your home.​

 

How much do a home visit cost:  If you have insurance that qualifies for a provider home visit, then we will bill your insurance provider.  If you do not have insurance or your insurance does not cover a provider home visit, then you may pay directly. Our service counties are:  Sumter, Schley, Macon, and Marion.​

 

How often can I be seen by a provider:  You and your Medical Provider will determine how often you will need to be seen by our home care provider.​

 

When will I be seen if I schedule an appointment:  Appointments are scheduled the same day or within 3 days.

 

​Is Community Care Mobile accepting new patients:  Yes, we are accepting new patients. You may call to schedule an appointment. You may also schedule an appointment directly from this website.

 

What services ae included in a home visit:  You will receive the same health care service from the provider as you would in a physical medical office which may include labs, and prescriptions. We may coordinate care with your health care provider also.​

 

Will Community Care Mobile replace my Primary Care Physician:  We can provide care for a single visit or we can assume the role of your primary care provider.

Can I see a provider in the office: Yes, you may schedule a visit in the office. We are open for in-person office visits, Sunday, Monday, Wednesday, Thursday from 5 pm-9 pm and Friday, and Saturday from 9 am-9 pm.

Deborah is a dedicated Family Nurse Practitioner from Americus, GA, whose passion for healthcare has shaped her career. She obtained her Associate of Nursing and Bachelor of Nursing Degrees from Georgia Southwestern State University, Master of Science in Nursing Degree with a Concentration as a Family Nurse Practitioner from Albany State University, and her Doctor of Nursing Practice Degree from Troy University.

​

Deborah brings a wealth of experience across various Federal, State and Private medical settings to include Intensive Care, Emergency Care, Urgent Care, Primary Care, Long-Term Care, Adult Health, Pediatrics, and Nursing Faculty. Since becoming a Registered Nurse and an ANCC Board Certified Family Nurse Practitioner, Deborah has committed herself to providing exceptional care to surrounding communities.

​

She is a member of various Nursing organizations to include American Nurses Association, Georgia Nursing Association, Nursing's Future Leaders, United Advanced Practice Registered Nurses of Georgia, and Southwest GA Nurse Honor Guard.

​

Outside of work, Deborah treasures hosting gatherings with family and friends, enjoys traveling creating lasting memories and indulges in her love for sewing which she started as a teenager.

​

Deborah created Community Care Mobile Clinic to bring healthcare directly to the homes of patients who have difficulty getting to and from their healthcare provider offices due to mobility issues or other medical problems. Community Care Mobile Clinic also offer In-Office visits and Telehealth visits.
